I suspect by spinning up the rear wheel (stuck in that hole) and delivering shock to the system in such a way that the body bound up on the RH wheel (thus pushing the body forward with a snap, far enough to break fastenings and leave the rear end on the tire).

Sounds like a plausible explanation. Those duallies make one hell of a flywheel. I don't see much damage near the wheel well, though. Must've been one hell of a bang, either way.

I'm pretty sure they jammed on the brakes and the rear axle hopped and broke free from the chassis or spring mounts and then rolled under the rear overhang... BOOM.

Wow, that's some video! I'd say that's what happened. The forum where I got this image said:
"This South Euclid ladder truck was heading down Wrenford Road April 4 when it came to an unexpected, abrupt and uncontrolled stop. The U-bolts that hold the differential to the leaf springs snapped, causing the differential to roll out the back. Several tow trucks came to the scene. Those tow truck operators considered several options while trying to figure out how to hook up the truck and load the broken parts. It took about four hours to clear the road."

Wheel hop is HELL on the chassis and suspension bits, whether it's under braking or acceleration.
Most road going vehicles don't generate enough torque under acceleration to tear themselves apart, but make can do so under braking.
